The data we collect about you
We currently collect and process the following personal data:
personal identifiers, contacts and characteristics (for example, name and contact details); and
information you provide to us as part of an enquiry which may include information relating to your physical and/or mental health.
We will need different information from you for different purposes which will be driven by your interaction with us. We will always keep the data we need to a minimum and ensure only those with a legitimate purpose have access to your data.
We will never share your data with anyone who does not need access without your written consent. Only the following people will have routine access to your data:
your practitioner(s) in order that they can provide you with our service; and
our administration staff, for example reception staff and book keepers will have access to basic information but do not have access to your medical history or sensitive personal information.
If you sign up to our newsletter via the website using your email address the information will be held by Wix and Mailchimp and used by Quester Global only for that purpose. Should you decide you no longer wish to receive the newsletter, please click the ‘unsubscribe’ button present in every email, or email your request to firstname.lastname@example.org.

Most of the personal information we process is provided to us directly by you for one of the following reasons:
as part of an enquiry related to the services we provide;
on signing up to the Quester Global newsletter using your email address; or

We use the information that you have given us in order to:
provide you with the best possible therapy;
keep you updated with news and information via our newsletter; and/or
collate data for research purposes to ensure the benefits of our therapies can become more widely recognised, but we will ensure all personal details are anonymised.
Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information are:
(a) Your consent.
You are able to remove your consent at any time but this may affect the service we can provide. You can do this by contacting email@example.com.
On signing up to the Quester Global newsletter using your email address you consent to us using that information for the purpose of updating you with news and information regarding the services provided by Quester Global.
In the event that you request therapy from Quester Global you will be asked to complete a consent form providing us with explicit consent to process your personal data which may include health data for the purposes of providing you with our services.
(b) We have a contractual obligation.
Your requesting therapy and our agreement to provide that care constitutes a contract. For us to provide our services and therefore fulfil the contact we will require your personal data.
(c) We have a legal obligation.
We have a legal obligation to retain your medical records for 8 years after your most recent appointment (or age 25, if this is longer), but after this period you can ask us to delete your records if you wish.
How we store your personal information
Your information is securely stored.
We keep details of your treatment and your personal details/health details for 8 years. We will then dispose of your information by shredding of paper information and deleting any computer files from our system.
Your data protection rights
Under data protection law, you have rights including:
your right of access - you have the right to ask us for copies of your personal information;
your right to rectification - you have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete;
your right to erasure - you have the right to ask us to erase your personal information in certain circumstances;
your right to restriction of processing - you have the right to ask us to restrict the processing of your personal information in certain circumstances;
your right to object to processing - you have the the right to object to the processing of your personal information in certain circumstances;
your right to data portability - you have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances; and
you are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
